Texas Medicaid: Help us recognize excellent attendant care

If you are a home- and community-based (HCBS) network provider, we invite you to nominate someone for the UnitedHealthcare Recognizing Excellence in Attendant Care in the Home (REACH) award. Originating in 2022, we present this annual award to exceptional attendants for their hard work, dedication and excellence in member care. 

Determining your nominee

Examples of excellence in attendant care include:

• 100% compliance in electronic visit verification (EVV) usage
• No complaints from any members
• Longevity/years worked with care provider
• At least 1 example of a situation where the attendant has gone above and beyond for a UnitedHealthcare member

How to nominate

You can nominate one attendant per year, using this simple process:

• Fill out the UnitedHealthcare REACH Award nomination form
• Email your completed nomination form to uhc_cp_prov_relations@uhc.com by Aug. 31, 2023

Recognition

We hold the UnitedHealthcare REACH Awards annually in each Texas Medicaid service delivery area (SDA). While only one person per SDA can win the award, we’ll recognize all nominated attendants with a certificate.
