Earn $150 for obstetrical risk assessment form

Your practice can now earn an additional $150 incentive for completing the obstetrical risk assessment form. This incentive applies to each of your pregnant patients who are UnitedHealthcare Community Plan of Mississippi members. This form helps us support your patients’ pregnancies alongside your medical care.

Next steps

To earn the incentive, complete the obstetrical risk assessment form for each of your pregnant patients within 15 days of the first prenatal visit. We only require 1 form per patient at the start of care.

You can submit the form by 1 of the following ways:

• UnitedHealthcare Provider Portal: Sign in to the UnitedHealthcare Provider Portal at UHCprovider.com and click the sign in button at the top right of the screen. Go to Clinical & Pharmacy in the top menu bar > Care Conductor and Notification of Pregnancy > Obstetrical Risk Assessment Form
• Fax: Print the form and fax it to 877-353-6913

After we receive the form, we’ll enroll these patients in our Healthy First Steps® maternal care program, which offers education, tools and support to promote healthy behaviors. The information you provide will help us offer your patients programs and resources that can be beneficial during and after their pregnancy.
