Texas Medicaid: Log ImmTrac2 consent to submit immunization records

Obtaining parent/guardian consent to submit a child's immunization record to the Texas Immunization Registry (ImmTrac2) helps:
- Health care professionals access important patient information
- Members access their immunization records for school, travel, changing providers, etc.

Next steps
To register children 17 years and younger, have a parent, legal guardian or managing conservator complete and sign an ImmTrac2 Minor Consent Form (C-7).
Then, log the member’s consent status on ImmTrac2 and retain original copy of the consent form in the member’s file.
