Arizona: Vaccines for Children renewal required

Health care professionals enrolled in the Arizona Vaccines for Children (VFC) program must renew their enrollment by Aug. 31, 2025. If you fail to re-enroll, you may be inactivated from the VFC program for non-compliance and members under age 18 may be removed from your panel. Please avoid disruption to your practice by completing your re-enrollment today.
