Maryland Medicaid: Enhanced continuous glucose monitoring systems coverage

As of Jan. 1, 2026, we expanded our coverage for continuous glucose monitoring (CGM) systems. This expansion includes UnitedHealthcare Community Plan of Maryland members with type 1 and type 2 diabetes who are using insulin or have gestational diabetes or problematic hypoglycemic events. 

Within 6 months prior to ordering the CGM, you’ll need to evaluate our member’s diabetes control and determine that the coverage criteria are met before submitting a prior authorization. 

How to submit and manage prior authorizations

• Sign in to the UnitedHealthcare Provider Portal with your One Healthcare ID
  • If you don’t have a One Healthcare ID, register now
• From the left-hand tabs, select Prior Authorizations & Notifications
