Prepare for these new Choice Plus network members

Beginning June 1, 2024, the Salvation Army – Southern Territory members will have access to the Choice Plus network. 

What this means for you

To view benefits, eligibility, prior authorization requirements and claim updates for these new members, please visit the UMR Provider Portal.

You don’t need to take any action during this transition. You can recognize Salvation Army – Southern Territory members by looking at the following member ID card:

Salvation Army – Southern Territory members will have access to the following Choice Plus network benefits:

• The national UnitedHealthcare network of health care professionals and facilities
• Medical utilization case management (UM) services, including notification, initial determination and inpatient care management
• The UnitedHealthcare Behavioral Health network
