New Mexico Medicaid: New Food is Medicine benefit

Our new Food is Medicine (FIM) benefit is now available to UnitedHealthcare Community Plan of New Mexico homebound members and pregnant members with diabetes. This benefit includes services that provide home-delivered meals and grocery boxes tailored to the member's medical needs.

 

New FIM benefits

Home-Delivered Medically Tailored Meals (MTM): MTM provides meals that are designed by a registered dietitian nutritionist (RDN) to meet the unique nutritional requirements of members with various health conditions. This service is available to UnitedHealthcare Community Plan of New Mexico homebound members and pregnant members with diabetes in New Mexico.

 

Home-Delivered Medically Tailored Grocery Boxes (MTGB): MTGB provides distributions of unprepared or lightly processed foods that members prepare at home. The contents are designed by an RDN to meet the unique nutritional requirements of pregnant members with diabetes. This service is only available to pregnant members in New Mexico. Homebound members do not have access to this program.

Complete the appropriate form and send equivalent information to nm_healthequity@uhc.com

 

Member benefit

The benefit includes up to 2 meals per day for the member (or the grocery box nutritional equivalent) for a duration of up to 11 months, not to extend beyond 2 months postpartum.

 

Member eligibility

Pregnant Medicaid recipients with diabetes diagnosis (type 1, type 2 or gestational). To be eligible to receive benefits for 2 months post-partum the benefit initially accessed while the member is pregnant.
