UnitedHealthcare Community Plan of Texas is partnering with Delfina Care (Delfina) to support earlier identification and proactive management of pregnancy complications.

Delfina is an AI-powered maternal health platform that can help you and your team deliver more informed, connected care to the pregnant UnitedHealthcare Community Plan of Texas members you serve. This optional service is now available to you and the expectant mothers under your care at no cost.

How it works

Delfina’s provider dashboard integrates with your electronic health record system to deliver predictive insights that help you identify who might need closer monitoring or earlier intervention.

From the dashboard, you can efficiently optimize care plans using advanced insights and automated workflows. The automated workflows are a standardized set of provider-approved clinic-initiated automated interventions that guide care teams in delivering timely, appropriate care while supporting provider efficiency and maintaining clinical oversight.

Members can also stay connected to you through Delfina’s app, which offers health education, and tools and support such as nutrition classes and lactation consultations. Members with clinical needs also receive remote monitoring devices to track blood pressure, blood glucose and weight between visits. Delfina’s non-clinical coaching staff support member adherence, education and attendance, reducing no-show rates and call burden.
