Nominate an outstanding attendant for the REACH Award

Are you a health care professional in the UnitedHealthcare Community Plan of Texas Home and Community-Based Services (HCBS) network? Help us recognize excellence in care by nominating an exceptional attendant for the REACH (Recognizing Excellence in Attendant Care in the Home) Award.

This annual award honors attendants who go above and beyond in caring for UnitedHealthcare Community Plan members.

Submit your nomination by Aug. 31, 2025.

Who is an attendant?

An attendant is a non-medical caregiver who helps individuals with daily tasks like bathing, dressing and meal preparation. This support allows people to live at home instead of in a facility. Learn more.

What makes a great nominee

Look for attendants who demonstrate:

• 100% compliance with Electronic Visit Verification (EVV)
• No member complaints
• Long-term service with a care professional
• At least 1 example of going above and beyond for a member

Award details

• You may nominate 1 attendant per year
• All nominees receive a certificate of recognition
• One winner is selected per Texas Medicaid service delivery area (SDA)
