What is the Housing Support Program?

Beginning July 1, 2025, Medicaid will pay for some housing-related services for eligible individuals enrolled in NJ FamilyCare/Medicaid. The Housing Support Program provides access to housing support services that help homeless or housing-insecure individuals find, obtain and maintain stable housing, which ultimately improves health outcomes. With this assistance, the integration of NJ FamilyCare/Medicaid funding and care coordination through managed care organizations will seamlessly deliver whole-person care while enhancing existing funding programs for the housing insecure.

  • Pre-tenancy and tenancy providers: If you are interested in becoming a participating provider, you can start the approval process by completing a credentialing application. For more information, please visit the Camden Coalition website.

    Completed applications can be sent by email to hcbsprovidernetwork@uhc.com. In the subject line, please state the organization’s name, Housing Supports – Application Submission.

  • Home modification and remediations providers: If you are interested in becoming a participating provider, please email hcbsprovidernetwork@uhc.com for more information or to request a credentialing application

    Please include the following in your request:
    • Subject line: Credentialing Application Request
    • Organization's name
    • Tax ID number (TIN)
    • National Provider Identifier (NPI) number
    • Housing supports services provided
       
  • Move-In supports: Providers who want to provide move in supports must also provide tenancy and/or home modifications and remediation services.  Please follow the credentialing process for your primary service tenancy or home modification and remediation. 
  • Housing supports initial assessment tool - Use this form is to request housing supports services and document eligibility. The form can be completed by a New Jersey FamilyCare member, parent, caregiver, family member, guardian, trusted friend or staff from an organization that helps a member.
  • Housing supports level of need assessment tool - This form is for a Housing Supports Program pre-tenancy and tenancy-sustaining health care professional who is authorized to deliver services to a member to:
    • Document eligibility for pre-tenancy and tenancy sustaining services
    • Request managed care organizations authorization and reauthorization for continued pre-tenancy and tenancy sustaining services
    • Classify individuals into 2 risk categories (lower level of need and higher level of need) to inform service provision and reimbursement under the state’s 1115 Medicaid waiver program
  • Housing supports housing stabilization plan - A Housing Supports Program is for pre-tenancy and tenancy sustaining health care professionals who are authorized to deliver services to a member.

    These health care professionals must create a housing stabilization plan following the start of services to support housing search and placement or immediate stabilization in current housing.

    At a minimum, you should update the plan once every 180 days during reauthorization requests for services or when member condition meaningfully changes, such as gains housing or loses housing, household structure changes or requires different services and/or level of need.
